Primary Duties and Responsibilities

The Health Ministries Outreach RN works within a specific area of Pastoral Care as Faith Community Nurses-FCNs serving as a spiritually-based health resource to the targeted community while promoting optimal bio, psycho-social, cultural and/or environmental dimensions of health. Provides administrative and development support to respective program including developing programs, policies, and evaluation. Identifies specific client, group or community needs, coordinates resources, and provides education to clients. Reviews progress to meet needs. Maintains required documentation. Maintains regulatory compliance standards. Performs other duties as assigned.

As a Faith Community Nurse (FCN), this role provides spiritually based care and outreach in a congregational setting. Uses population health concepts to identify needs including coordinating health education and screenings; providing health counseling, advocacy and education, developing support groups, etc. Interacts with people of many faith traditions. Participates in Flu Vaccination clinics. May require frequent weekends and some evenings.
